How To Receive Copies Of Your Medical Records
To request your medical records from the Encino Hospital Medical Center, please completely fill out the authorization form (English or Spanish) and provide a copy of your valid photo identification. You may send the documents to us by mail or via fax at (818) 907-2813.
Our office is open for records request Monday through Friday from 8:00 a.m. to 3:30 p.m. The Medical Records or Health Information Management Department is located in the Encino Hospital Medical Center, 16237 Ventura Blvd., Encino, CA 91436. They can be reached by telephone at (818) 907-4525.
Please note: If your medical records contain sensitive information as listed in Section B of the authorization form, please check the appropriate box and initial next to the box.
For Other Related Requests such as Diagnostic Images/X-rays: Health Information Management does not process requests for copies of diagnostic images, such as X-rays, MRI scans and CT scans. If you need to get a copy of your diagnostic image, please contact the Radiology department at (818) 995-5245.
