Encino Hospital Admitting Department staff are here to provide you with
ongoing support to assist your patient in working with their insurance clinician to ensure coverage for their inpatient and outpatient treatments.

The hospital may also be able to provide financial discounts for patients without full insurance coverage. For consideration, please fax the following information to the admitting department for insurance verification:

1. Patient information (including date of birth and Social Security number
2. Copy of insurance card (front and back)
3. Procedure name and code(s)
4. Diagnosis
